Indiana Home and Community Based Services (HCBS) Assisted Living Medicaid Waiver Program
Jewel Place Senior Living is licensed by the Indiana State Department of Health as a Residential Care Facility (RCF) and is Medicaid Waiver Certified by the Indiana Division of Aging/Office of Medicaid Policy and Planning (OMPP) and Indiana Medicaid and participates in Indiana’s Home and Community Based Services (HCBS) for Assisted Living.
Jewel Place Senior Living participates in Indiana’s Pathways for Aging program and has contracts with Anthem Blue Cross Blue Shield, Humana and United Healthcare to provide Assisted Living services. Indiana Pathways for Aging is a statewide coordinated care program for Indiana’s Medicaid enrollees who are 60 years of age or older.
Jewel Place Senior Living can immediately accept individuals who are Indiana Medicaid Waiver approved. If an individual is not currently Medicaid Waiver approved, no worries!
